It’s easy to join.  Just print out this form, complete the information below and bring it in to any Shoe Carnival store.  The cashier will give you a Savings Club membership card and you begin earning points toward your first reward.

Here’s how the Savings Club program works for you:

• For every penny you spend, you get a “Point.” 
• When you accumulate 12,500 Points, we’ll send you a $5 Reward Certificate you can bring in and spend like cash.
• Plus, you’ll get 1,000 extra club Points the day you enroll.
• You’ll get periodic members-only special offers throughout the year and notices of special sales.
• You can always check your Point balance, reward status and update your personal information at www.shoecarnival.com.  Just look for “Savings Club” at the top of our home page.
